We gladly accept the insurance plans listed below.

For those patients who do not have health insurance or those who have maxed out their benefits, we offer a private pay plan. Please contact Zone Physical Therapy today for more information on our fees for service and payment details.

We have included most of the major plans with which we are in-network. We recommend that you verify with your health plan what physical therapy benefits you have available. Please call the office to verify other insurance plans. Zone Physical Therapy offers a $175 initial visit and $125 per subsequent visit, a self-pay fee if we are not in-network with your insurance carrier. If you are out-of-network, we can provide an itemized receipt for you to submit to your insurance carrier.
